Our referral process

To refer your patient, simply fill in the online form or download the pdf and print and we will be in touch. For more information, or if you would like to discuss your referral further, please contact us – we’re always happy to help.

We accept referrals for the following treatments:

• Specialist orthodontics at TSS – Download & print PDF
  (Usually £55 fee, currently complimentary)
• Endodontics at LHDP – Download & print PDF
  (£75.00 assessment fee)
• Dental implants at LHDP – Download & print PDF
  (£175.00 assessment fee)
• Oral surgery at LHDP – Download & print PDF
• Restorative & Prosthodontics at LHDP
  (£175.00 assessment fee)
• Cosmetic treatment / Composite bonding
